Download and print the form, or check with your local Laverty collection centre as some may accept the form via email
Fast for 8 to 12 hours before your blood draw (water is fine). Do not exceed 12 hours of fasting. This means if your blood draw is at 8am, your last meal should be no earlier than 8pm the night before.
Take your form to a Laverty collection centre to have your sample taken
Once your results come through to me, we book a follow-up consultation to review them together and map out your personalised reset plan
